The Foundation Begins

As of 2019, I was blessed to develop my corporation which is a charitable nonprofit Foundation:

My Journey

Denise Moss, CEO and Founder of Denise Moss Connected Hearts Foundation shares her powerful and compelling story of how this Foundation began:

“In 2013 I was diagnosed with Dilated Cardiomyopathy and Congestive Heart Failure.
On my birthday February 5, 2016, I found out something amazing. I found out that I was approved by the ‘committee’ to be on the national organ transplant waiting list to receive my new donor heart…

I drove myself to the ER on the afternoon of Friday November 25, 2016, the day after Thanksgiving, after procrastinating whether I should go in due to dealing with a chest pain that I’d been having all day.
They admitted me instantly because it was protocol for someone waiting on an organ. I was admitted on Friday November 25, 2016 and was kept for a couple of nights for observation. Sunday November 27, 2016, the heart transplant team walks in and tells me it ‘is now time for’ my heart transplant to happen. I needed a sudden heart transplant!
On Tuesday November 29, 2016, I received a phone call on my cell phone while I was in the hospital, by the national organ transplant waiting list maintained by The United Network for Organ Sharing (UNOS) stating that my donor heart was in and for me to be prepared for my heart transplant surgery tomorrow morning on Wednesday November 30, 2016 at 5:30 am!!!!
Early the next morning on November 30, 2016, I had a miraculous and successful heart transplant surgery where they removed my entire heart that I was born with out of my body and put in my new heart from a donor. Wow. That’s so amazing!”

Present Day

After my heart transplant, I wanted to be a blessing to adult individuals with cardiac illnesses and heart transplant recipients. I wanted a foundation to serve as a platform to give preventable and practical resources on lifestyle heart-healthy practices such as exercise, and choosing healthier meals, providing information about heart illnesses, providing emotional support teams to individuals during a cardiac crisis and/or after a heart transplant, and providing financial support for adult individuals after a heart transplant which has impacted their household, with a focus on underserved communities and single-parent households during these delicate times.

I also wanted to become a voice for the underserved communities for those individuals with a cardiac illness but have been diagnosed with something else in error.  I’ve personally been misdiagnosed twice. The DMCH Foundation will also serve as a platform to bring community awareness by promoting for more organ and heart donors. By choosing ‘Yes’ on your driver’s license or ID and by registering online with organdonor.gov, you can help to save many lives.

It is my purpose, and my belief, that this miraculous heart transplant journey that I am living will truly help to catapult your faith.

DMCH Foundation’s goal is to help you realize your inner strength and purpose for your life. Lastly, our aim is to encourage the research of Heart Transplantation and with the support of a healthy lifestyle and a positive outlook, a healthy heart can be achieved.
